Any advice for restarting over and over cause I can't stay motivated anymore?

Little backstory. Was 255lbs at 17, lost 40 pounds in a few months and felt at my best. I slowly gained back this weight over the next 3 years and putting on more than I originally started with. I am now 20, 6'4, 270 pounds. I am a very broad and 'built' guy so I look like a rugby player with my frame. Behind all my clothes I am insecure and I hate how I look. I am in the gym, I play sports but the hardest thing for me is just staying on track with my calories. I either sneak something in and don't think much of it till i realize I lost all the progress I made in a few days/weeks. I just want the focus and motivation I had when I started.

I really just want to know how some of yous manage to stay on track. This is probably the hardest mental fight I have to deal with and I just wish something would snap. I see my body every time I shower but everytime I say I will do something and end up doing nothing. Any tips?
